Job Application Question-Help!

Hi, I was employed for a whole year. My first 3 months I had a supervisor. He left because he got married. We never got a replacement supervisor. Im filling out an application and they're asking for a supervisors name and number. But I have no one to list. What do I do?

Who was the next person up in the hierarchy?
Simply provide the same explanation as you've given us. Write "Please refer to accompanying letter" in response to the question on the form..

Then, in your covering letter, write "I note that your application form requires the name of my supervisor during my employment with Bloggs Services Ltd. Mr David Jones was my supervisor from April 2018 to June 2018. Mr Jones then left the company and no replacement supervisor was appointed to the role throughout the rest of my service with the company".
I'd get back to that workplace and do my best to find their name and number. But either way, it wouldn't hurt to go to the new employer and ask them nicely what they think you should do. I hope this is in any way helpful, best of luck!!
In the Civil Service, references come from HR. Supervisors can give an opinion but only in a personal not official capacity.

Have a word with your old employers to see what you should do.
