That is so beautiful QM - the only one I know is from the sermon by Canon Scott Holland:
'Death is nothing at all. It does not count. I have only slipped away int the next room. Nothing has happened. Everything reamains exactly as it was. I am I, and you are you, and the old life that we lived so fondly together is untouched, unchanged. Whatever we were to each other, that we are still. Call me by the old familiar name. Speak of me in the easy way you always used.
That's a part of it - bit too long to write out in full.
Suzanne, I'm sure the piece mentioned by Cetti is the one you are actually looking for. I knew there was something along these lines somewhere, but just couldn't put my finger on it. If you click http://www.merseyworld.com/faith/html_file/magmay2
000.htm a link will take you to a web-page with the whole piece on it. Just scroll down to the heading with Canon Scott Holland. (Well done, C. My reference is OK, but it's not the one required, I'm sure. I hope you don't mind my providing the link.)

Death is nothing at all,
I have only slipped away into the next room.
I am I, and you are you,
Whatever we were to each other we are still.
Call me by my old familiar name,
Speak to me in the easy way which we always used.
Put no difference into your tone,
Wear no forced air of solemnity or sorrow,
Laugh as we always laughed at the little jokes together.
Play, smile, think of me,
Let my name be ever the household word that it always was,
Let it be spoken without an effort,
Without the ghost of a shadow on it.
Life means all that it ever meant;
It is the same as it ever was.
There is absolutely unbroken continuity.
What is death but a negligible accident?
I am but waiting for you
For an interval, somewhere very near,
Just around the corner.
