BEDAZZLED

I have been playing a silly little game called bedazzled, keeps me occupied and out of mischief. But just recently i have been asked to register with username ect. I can't work out how to do it. Sounds silly I know, but there are no instructions as to what to put in what box!!!! Can anyone help a confused simple gamer!
