Yep, the grammar's ok but the report's not necessarily accurate.
Patient B died, so it's not worth all the debate and heated comments on the grammar expressed.

After all, this is the NHS that we are talking about and the elimination of punctuation saves, on average, 0.015% of a nurse's time usage in any given day.
