You drill a small hole in the centre of the offending bolt and insert the extractor (which looks a bit like a tapered drill but with a coarse left hand "thread"). Turning the extractor (either with a decent pair of grips or, preferably, a die-holder) will do the job. A bit of WD40 or similar before you start should ease the process.
