Dear People,

I am looking for a forum where I can questionnaire with my colleagues. Suppose I upload a document which needs to filled in by my colleagues in such a manner that the filled in documents should be visible only to me but not visible among my colleagues.

I want to be the administrator of the forum and my colleagues can only fill in the docs.
I should be able upload the questionnaire on the forum and my colleagues can download, fill in and re upload the filled form or even fill in online mode(without downloading the doc).

The most important thing is that their answers should only be visible to me and the particular colleague.

Kindly let me know what platforms can I use for this purpose.
