It wasn't.
The gun was pointed at the camera, in order for the camera to film the gun firing.
The cinematographer was standing near the camera and was killed by shrapnel, I believe, from the camera which was hit by the live round.
Had the gun been loaded correctly, it would have fired, with the requisite flash and report, but no projectile emerging, and no damage or injury.
