For more on marking an answer as the "Best Answer", please visit our FAQ.You can do it by deed poll on any of the deedpoll sites on the net, (like I did) and it cost round about �25. (run a google search... I used deedpoll uk, if I remember!
They'll send you the paperwork...and certificate.
Once you have this certificate, if you enclose it with a passport application for eg, they will use it as proof that her name has been changed.
You then need to inform all the agencies that have her registered, GP, School, Dentist etc, and ask them to send you an acknowledgement... so you know they've changed their records accordingly.
As you weren't married, you don't need to ask permission.
Alternatively, you can drop by the registry office where her birth was registered and speak to someone there, and they can do it for you.
