I am not sure that the Republican Party has entirely moved on from that attitude. More broadly it's probably something to do with fear of big government, states' rights v. Federal control, etc...
jim have you seen this film? Moore is the US version of a left winger and no doubt you and others on here would find it strange that I ally with him on this subject but really I have huge sympathy with the subject:

This is the land of the free( where some are freer than others) if you are white middle class & have plenty of dollars welcome. YMB.......it was a difficult thing here in Britain, in 1948 but Aneurin Bevan still managed to get it to happen.
I appreciate that Britain was, and still is, a much smaller country than America. But America is immeasurably wealthier than we are, and could just as easily have managed something similar.
We were completely broke after WW2, and yet it still happened. Bevan was once asked how he persuaded the Doctors to go ahead, because without them it would never have got off the ground.
He replied that "he filled their mouths with gold"

Anyone can, and does, claim to be best, etc. regardless of any evidence against the claim - the point is that saying it brashly enough, loudly enough, often enough for long enough actually succeeds in convincing lots of people. In spite of the appalling inequalities, cruelty, violence, etc. in the USA, millions still try to reach the place (legally or illegally) thinking their lives will be trouble free once they do. The USA is but one example of similar claimants, sometimes/often including (by people in) the UK.
I gave up on even visiting the USA when I discovered that even in the event of a terrorist attack like 9/11 the free emergency medical treatment is limited to stabilizing your condition ONLY.
In other words if you are unfortunate enough to have a leg blown off by a terrorist bomb, the US Emergency health service will stop the bleeding and dress the wound only! All else is up to you to pay for.
As almost no health insurance policy will include injury due to terrorist or criminal acts you are st***ed .Thousands of people who have been injured in criminal or terror attacks have been bankrupted by the medical bills they have had to pay.
